Additional Online Appointments
The Practice has recently opened up more on-line appointments - please ensure that you book into the appropriate appointment slots AND MUST CONTAIN A REASON FOR BOOKING - if appointments are booked inappropriately or without reason given, patients may not be seen and will be asked to rebook.
As from the 1st June 2021 the prescription office will only be open from 11.00am - 1.00pm Monday - Friday
If you have an urgent prescription query outside of these hours then please speak to a receptionist
Dementia Friendly Surgery
Oaklands Surgery has won the top award for being a Dementia Friendly GP Surgery for implementing changes within the surgery to improve support for the care of people with dementia and their carers. The award was presented to Dr Mujahid in September 2018 at a GP training session.

Booking an appointment
When booking an appointment online through the NHS app or website please make sure you do the following.
- State the reason for appointment
- Please look on our appointments tab at the top of the page to make sure your booking the appropriate clinicians for your complaint.
- State if the appointment will be (F2F) Face to Face or (Tel) Telephone appointment

WE ARE A GP TRAINING PRACTICE
At Oaklands Surgery we are proud to have become a GP TRAINING PRACTICE.
GP's Partners at the Practice are qualified to offer training for Doctors wanting to become new GP's. This means they have had special training covering all aspects of the GP training process with an emphasis on teaching consultation skills. The GP trainees see patients initially at a slower rate and build up gradually so that they can develop these skills teaching but also ensure that we are happy that our trainees are all safe and competent doctors and that our patients are in good hands!
The good news for patients is that being a Training Practice helps to ensure that we, as GPs are up to date and fully informed. As Trainers we get regular assessments from the training board to check that the standards for training are met and that the Practice is a suitable learning environment for our trainees.
Information Sharing
Health and care organisations have taken great strides in bringing together health and care services to improve the experience for individuals.
As part of this work, we want to ensure that health and care professionals directly involved in a person’s care have access to the most up-to-date information about them.
My Care Record facilitates the lawful sharing between health and care professionals through access to records by participating partners.

ACR project for patients with diabetes
The data is being processed for the purpose of delivery of a programme, sponsored by NHS Digital, to monitor urine for indications of chronic kidney disease (CKD) which is recommended to be undertaken annually for patients at risk of chronic kidney disease e.g., patients living with diabetes. The programme enables patients to test their kidney function from home. We will share your contact details with Healthy.io to enable them to contact you and confirm that you wish them to send you a test kit. This will help identify patients at risk of kidney disease and help us agree any early interventions that can be put in place for the benefit of your care. Healthy.io will only use your data for the purposes of delivering their service to you. If you do not wish to receive a home test kit from Healthy.io we will continue to manage your care within the Practice. Healthy.io are required to hold data we send them in line with retention periods outlined in the Records Management code of Practice for Health and Social Care. You can stop your confidential patient information being used for research and planning and can now OPT out of this up until the 1st September 2021. if you would like to do this please contact the surgery and we can mark this on your record.
If you're happy with your confidential patient information being used for research and planning you do not need to do anything.
Any choice you make will not impact your individual care.
